This food diary is the best I have found. 
I have been through a lot of different ones since starting a healthy eating plan 2 years ago and this one is by far the best I have found! 
There is space for the date, eating plan, weight and measurements at the top of each page. Then there is a very well laid out grid with the headings; free food, fruit/veg, dairy, fibre and guilty pleasures for the 3 meals of the day plus snacks. Then finally at the bottom of each page there is a 'quick reference guide' with a tick list for guilty pleasures eaten, water intake and fruit/veg eaten. Also a space for amount of exercise done and how you felt the day went. 
Personally this is everything I need from a food diary.
