Instructions on the box are very clear. My micro-sim fitted exactly in the metal frame. Insert the metal frame into the cutter and cut. Takes about the same pressure as a stapler. Cuts Nano Effortlessly

This is a nice little tool. It will cut a standard sim down to Nano size, or a micro sim down to Nano size. You will end up cutting some of the metal when you punch it out, but it will still work. If you need your sim to be micro size, you will still have to cut it to Nano size and then put it in one of the included adapters. You may also need to sand it a little with the included piece of sandpaper. Be sure to have the sim # recorded somewhere before you go sanding it away.
