Habilitationsschrift aus dem Jahr 2002 im Fachbereich Medizin - Chirurgie, Unfall-, Sportmedizin, Universitat Ulm (Chirurgische Klinik), Sprache: Deutsch, Abstract: Nichtsteroidale Antiphlogistika haben einen mehr oder weniger hemmenden bzw. verzoegernden Einfluss auf die Knochenbruchheilung. Auch wenn dies bisher nur im Tierversuch eindeutig nachweisbar war: Alle vorliegenden Ergebnisse sowie die bisher gefundenen Wirkmechanismen lassen keine andere Schlussfolgerung zu als die - zumindest bedingte - UEbertragbarkeit dieser Erkenntnisse auf den menschlichen Organismus. Entwickelt wurde ein praktikables Frakturheilungsmodell an der Rattentibia, was sich - je nachdem ob zusatzlich eine Fibulafraktur gesetzt wird oder nicht - als stabiles oder instabiles Modell anwenden lasst und auch beide Modelle untereinander vergleichbar sind. Bei der oralen Medikamentenapplikation konnte gezeigt werden, dass diese in der Zubereitungsform von Geleekugelchen von den Ratten problemlos aufgenommen wurde. Hilfsmittel wie Magensonden o.a. waren hier nicht erforderlich. In dieser Untersuchung konnte die Hemmung der Knochenbruchheilung unter Einnahme von Diclofenac mittels biomechanischer Testung eindeutig nachgewiesen werden. Diese Ergebnisse konnten bestatigt werden durch die radiologische Knochendichtemessung sowie die histologische Auszahlung neu gebildeter Osteoblasten in der Spongiosa. Wenn auch diese Ergebnisse nicht in allen Punkten histologisch verifiziert werden konnten, so durfte dies lediglich an der Anzahl der operierten Tiere liegen, da die Unterschiede zwischen den einzelnen Gruppen in Relation zur Gruppengroesse zu gering waren. Eine zeitliche Einordnung ist histologisch insofern moeglich, als sich die Tiere der Placebogruppe in einem fortgeschritteneren Stadium der Heilung befanden verglichen zu den Tieren der Diclofenacgruppe. Somit kann man eindeutig von einer Verzoegerung der Knochenbruchheilung sprechen. Auch die Wundheilung wurde durch Diclofenac verandert. Die Einwand
