By accepting our service, you (the buyer) agree that:
1) If necessary you will check with your dentist before using the products.
2) If you experience any problems with our products please stop using and consult your dentist.
3) Use our products for the sole purpose of what they are intended.
4) Will not use the product if you are pregnant.
5) Will take the impression yourself, of your own teeth.
6) Will not use if you have loose or decayed teeth.
7)Will not use if you have any crowns, bridgework, veneers, fixed orthodontic treatment or other oral problems e.g. gum disease.
8) Understand that employees/staff at Custon and Xsmiles are not dentists or registered professionals but experienced technicians.
9) Will not use if allergic to carbamide peroxide, glycerin, carbome, flavor, hydrogen peroxide, ammonium, hydroxide, vinyl polysiloxane(vps) and silicone.
10)You agree that all warranties and returns outside of our normal terms and conditions are the sole discretion of the company and that total liability is limited to the amount paid for the service.
11)You will remove any dentures before impression taking.
12)Accept that if the impression of your teeth is not acceptable or a mistake is made whilst using the impression putty then extra impression putty will be purchased by yourself.
13)You are responsible for checking the item upon receipt. If an item has been misplaced or in the unlikely event and item is missing you should contact us within 1 week of purchase and we will replace any required item(An additional cost may occur). If we are not contacted or contacted outside this time you accept full responsibility and if required purchase the item again.
14)You should check the fit of the impression trays prior to taking the impressions. If you feel that none of the impression trays are a good fit you should not proceed with taking the impressions and return all items unused.
15)Impressions must be returned and received by us within 3 months of original purchase date(This includes any new impressions that are required). Please allow sufficient time for any required new impressions to be retaken and returned. You are responsible for ensuring any new impressions required are returned within this return period.
16)If you feel for whatever reason you cannot return the impressions within the stated return period please notify us and return the impression kit unused. We must be notified within this 3 months and the impression kit must still be returned within the 3 month period.
16)If the impression are returned late your order will be void and you will not receive a refund and if required purchase the item again.
17)In the event that an item is missing from the order, if replaced the impression return period be start from the original purchase date and not from from the date of contact or receiving the replacement item.
18)The price paid is for the impression kit and the trays to be completed within the quoted 3 month return period. You accept that to ensure all customers are treated equal under no circumstances will we accept any impression returned late eg, illness, covid and you accept full liability for these rare events.
19)If a new impression is required your order will be placed on hold and any impressions kept for the remaining time of our return period from the date we received your impressions. If we do not receive a new impression within this time period your order will be cancelled and no refund will be provided.
20)If only 1 impression of any set needs retaken we will await the new impression before proceeding with the order and will not process each tray separately.
21) If we explain that your impressions or not correct and we are requested to make the trays, if there are any issues you will not be entitled to refund..
22)You are aged 18 or above.
23) Any additional product changes / amendments if, in the judgment of customandxsmiles ” will be necessary to improve the comfort and fit of the product, eg. the tray may not cover all teeth if they have not fully recorded in the impression.
24) You are responsible for the return postage of the impressions and we(customandxsmiles) are not liable for any impressions missing in transit. If impressions are not received a new kit will be purchased by yourself or you accept that you will only be entitled to a partial refund.
25)Any spare putty is included free to be used before returning the impressions. If it has been returned and a new impression is required the will be a charge for it to be returned to retake the impressions.
26) If the putty has been opened or the seal has been broken or tampered with you will not be entitled to a full refund.
CUSTOMER CONSENT
I hereby request and authorize customandxsmiles and whomever they may designate as their assistant(s), to manufacture (from an impression that I take myself) a custom made dental product to fit over my existing teeth for cosmetic, sports or personal protective applications only.
1. I, on behalf of myself and my heirs, executors and administrators hereby release any and all claims, rights, liabilities and causes of action, known or unknown, that I may have against the above customandxsmiles and his/her staff, officers, directors, employees, successors and assigns arising from or related to the manufacture, supply, development and self- installation of a custom made dental product known as (NightGuard, Mouthguard, Gumshield, Teeth Grinding nightGuard, Dental Retainer, or Teeth Whitening/ bleaching Trays / Kits) for my own personal dental use .
2. I authorize any additional product changes / amendments if, in the judgment of customandxsmiles ” will be necessary to improve the comfort and fit of the product.
3. I understand that I must follow the instructions carefully and fully to return an adequate impression(s) of either my top and / or lower teeth that the lab technicians can work with to produce a product that fits. I understand that should I not follow the instructions carefully nor produce an adequate impression then I may incur an additional charge for a replacement impression kit.
4. I understand that, although unusual, unexpected complications or less than desired results can occur. I understand the customandxsmiles is strictly making custom fit product to fit the model of the impression(s) I provided and that I may not be satisfied with the final result.
5. I understand that this process may take between 10 and 30 business days (or more) from the date usable impressions are received. I understand that customandxsmiles does not guarantee any delivery date and is not sensitive to my life events.
6. I understand that excellent home care techniques, using a variety of aids, may add considerably to the successful outcome of my custom dental product, and I understand it will be important for me to follow the home care
The seller and manufacturer of this product assume no responsibility for any usage of this product. If you don t agree to or don t understand any of the above terms, you must not purchase this product.
Our Returns Policy We can only accept returns of any unused and unopened impression kit that is not suitable or has been ordered in error(If the putty has been opened or the seal has been broken or tampered with you will not be entitled to a full refund.). Once an impression kit has been used you will not receive a full refund and the cost of this kit etc. and any other costs we have incurred will be deducted from any subsequent refund. We cannot accept a return of a final custom fit product unless it is determined that the return is due to a our error or Product defect. If there is any issue with your custom item i.e. regarding the fit etc the item in question should be returned prior to any remake or refund. You must contact us and return the item within 1 week of the delivery date. If you wish to return your order, please notify us in writing (email to: customandxsmiles@outlook.com is acceptable) Please include a detailed description of why you are returning your order complete with pictures and/or video documenting any issues. All original postage costs incurred by Custom and Xsmiles will be deducted from any subsequent refund. Buyers are responsible for all return shipping costs. We advise all returned products to be sent recorded or signed for delivery as we cannot be held responsible for any returns lost in the post. If the return is due to an error by Custom and Xsmiles, shipping charges may be refunded. Returns which are used, incomplete or missing components may not be accepted. Our Refund Policy ‘We’ hope ‘You’ will be happy with ‘Your’ purchase. However, should ‘You’ feel it necessary to return an item, we aim to make the returns process as simple as possible. If ‘You’ are unhappy with an item, simply return it in the original packaging within 30 days of dispatch and ‘We’ will give ‘You’ a refund or replace the item. EXCLUDES any custom made product where an impression kit has been received. If ‘You’ have any problems with ‘Your’ Custom and Xsmiles custom made product we will endeavour to re-place or re-make your custom made product up to one (1) time (additional photos and/or impressions may be required). Please note that this is judged on a case by case basis and is strictly at the discretion of Custom and Xsmiles. We begin work on your custom made product once we receive your impression(s), once this process begins there are NO MONEY BACK REFUNDS except where prohibited by Law.
You are liable for the home care of your item and should not undertake and any action that would result in the damage i.e. using warm water to wash.
