After notification and a couple of questions back and forth, the seller sent the missing items right away.

Drive center had a burr around the set screw hole but I was able to take care of it with some sandpaper on a dowel (and seller offered to replace it if that didn't work); also the set screw is not the standard Shopsmith size (comes with the right wrench); but much sharper than my old rusty one.
